This child-account exclusion card helps payers flag invalid dependent billing entries on official printed invoices. It works across all three supported desk categories: cryptocurrency tax dependent sub-accounts, enterprise cloud child resource accounts, and dental insurance dependent minor patient accounts. The template below standardizes exclusion requests so billing administrators, tax preparers, and insurance carriers can process corrections without follow-up requests for missing data. You can store all associated documentation in your Field Ledger folder to cross-reference if the correction is flagged for secondary review. Exclusion card signature box
The signature box should be placed at the bottom of the front page of the exclusion card, with printed name fields and date fields below each signature line. The third signature belongs to the party processing the adjustment: for cryptocurrency tax requests, this is your assigned tax preparer confirming the excluded transactions do not qualify for reporting on your return; for enterprise cloud invoice requests, this is your accounts payable lead confirming the excluded charge was not allocated to any departmental budget; for dental insurance claim requests, this is your plan administrator confirming the patient listed does not appear on your active dependent roster. All signatures must be dated no earlier than the date you compiled all supporting documentation for the request, to avoid invalidation due to stale attestation. Close the letter with a numbered list of all supporting documents you are including with the request, and a contact phone number and email address where you can be reached within 2 business days if the processing team has questions. If you are submitting the request via email, include the cover letter as the body of the email, and attach all supporting documents as password-protected files if they contain sensitive personal or financial information. The subject line of the email should include your account number, the billing period, and the phrase “Child Account Exclusion Request” to ensure it is routed to the correct processing queue.
