You will need a copy of your signed prepay agreement, all monthly invoices issued for the SKU in the 12-month coverage term, and access to your full payment history files to complete the reconciliation process accurately. Annual prepay schedule summary column

The annual prepay schedule summary column is a verbatim transcription of the credit allocation terms outlined in your signed prepay agreement, with no modifications or adjustments permitted unless a fully executed addendum to the original contract is included in your tracking folder. For each billing period, you will enter the exact dollar amount of prepay credit that the agreement states should be applied to that month’s invoice for the single SKU in question, alongside the unique SKU identifier, contract ID number, and effective date of the prepay agreement. If you are tracking a cryptocurrency tax service SKU, this column reflects the agreed monthly credit allocation for the single wallet or transaction tier you prepaid for at the start of the tax year. For enterprise cloud SKUs, this column matches the reserved instance or dedicated storage credit allocation defined in your annual prepay contract with your cloud provider. For dental implant insurance SKUs, this column lists the monthly credit allocation for the specific implant-related procedure you prepaid for as part of your annual policy premium. You should cross-verify all entries in this column against your original signed agreement at least once per quarter to ensure no transcription errors have been carried over during monthly reconciliations, as errors in this column will lead to incorrect variance calculations later in the term.

Monthly invoice line-item entry box

This section is reserved for verbatim transcription of line-item details for the single tracked SKU from each monthly invoice issued during the 12-month prepay term. Only include the line item that corresponds exactly to the SKU listed in your prepay agreement, and exclude all unrelated charges including late fees, one-time service add-ons, separate SKU purchases, sales tax (unless explicitly included in the prepay agreement terms), and administrative fees that are not tied to the core SKU cost. For each invoice, you will enter the invoice number, issue date, total billed amount for the tracked SKU, the exact amount of prepay credit listed as applied to that SKU on the invoice, and the net amount you were charged for the SKU after credit application. If the tracked SKU is not listed on a monthly invoice, mark that billing period’s row with a flag for follow-up, as this may indicate a billing error, unapplied prepay credit, or misclassification of the SKU on the provider’s side. For dental implant insurance claims, you will also enter the claim number associated with the month’s service, if applicable, to cross-reference against your carrier’s claims database. For enterprise cloud billing, you will enter the cost center tag associated with the SKU to align with your internal finance tracking requirements. For cryptocurrency tax records, you will enter the wallet ID or transaction tier associated with the SKU to support year-end tax filing documentation.

Single SKU purchase tracking folder

All documentation related to the prepay agreement, monthly invoices, payments, and adjustment requests must be stored in a dedicated single SKU purchase tracking folder, organized to support audit requests, tax filings, reimbursement claims, or billing dispute resolution. The folder should include five labeled tabs, arranged in the following order: first, the signed original prepay agreement and all fully executed addendums, with all credit allocation clauses highlighted for easy reference; second, all monthly invoices for the 12-month term, sorted by issue date, with the tracked SKU line item highlighted on each invoice; third, payment receipts for both the initial prepay amount and any additional monthly payments made for the SKU after credit application, sorted by payment date; fourth, all quarterly billing adjustment request forms submitted during the term, alongside supporting documentation and written responses from your provider; fifth, quarterly and end-of-term reconciliation notes, including a copy of the completed cross-reference table. You can use Field Ledger’s digital folder templates to organize these materials if you prefer cloud-based storage, but always keep an offline encrypted backup of all files to avoid data loss in the event of a service outage or account access issue. For cryptocurrency tax records, this full folder will be submitted to your CPA or enrolled agent as supporting documentation for your annual tax filing. For enterprise cloud expenses, the folder will be shared with your internal finance team for annual expense reconciliation and budget forecasting. For dental implant insurance claims, the folder will be submitted to your carrier to verify prepay allocations and support reimbursement requests for covered services.

Quarterly billing adjustment request form

Every quarter, after you have entered all monthly invoice line items for the preceding three months into the cross-reference table, you will complete a quarterly billing adjustment request form if the total variance for the quarter exceeds the minimum threshold defined in your prepay agreement. The form must include the following mandatory fields: unique SKU identifier, prepay agreement contract ID, quarter covered by the request, line-item variance details for each month in the quarter, total net variance for the quarter, specific adjustment requested (credit to future invoices, refund of overpaid amounts, or correction of underapplied credit), references to supporting documentation including invoice numbers, payment receipt numbers, and relevant clause numbers from the prepay agreement, and a formal signature or digital authorization from the account holder of record. You must attach copies of the relevant monthly invoices, the excerpt of the prepay schedule covering the quarter, and payment receipts for any additional amounts paid during the quarter to the form before submission. Log the submission date and method (email, online portal, mail) in your tracking folder, and follow up with your provider if you do not receive a written response to your request within 30 days of submission. Do not make unilateral adjustments to your monthly payments based on calculated variances, as this may result in late fees or service interruptions unless your provider has formally approved the adjustment in writing.

Payment variance cross-check reconciliation note

At the end of the 12-month prepay term, after all monthly invoices have been entered, all quarterly adjustment requests have been resolved, and all final payments have been processed, you will complete a payment variance cross-check reconciliation note to finalize the file for archival or submission. This note will include the following details: total scheduled prepay amount per the original agreement, total amount of prepay credit applied to monthly invoices over the full term, total additional payments made for the SKU during the term, total invoiced amount for the SKU over the term, net end-of-term variance (a positive value indicates you are owed a refund or future credit, a negative value indicates you owe a final payment to your provider), confirmation that all quarterly adjustment requests have been resolved and all agreed-upon adjustments have been applied to your account, and a note indicating the final disposition of the file (archived for audit, submitted to tax preparer, submitted to finance team, submitted to insurance carrier). Attach a fully completed copy of the cross-reference table to this note, alongside a copy of all finalized adjustment request responses, to create a single unified supporting document for any future inquiries related to the SKU’s billing over the 12-month term.
