The covered vs uncovered table organizes savings-plan row data to separate eligible coverage from ineligible usage claims. It is designed for use across three core savings plan categories: cryptocurrency tax cost-basis savings plans, enterprise cloud committed use savings plans, and dental implant insurance benefit savings plans. Table Coverage Column Entries for Verified Savings-Plan Rows

All verified coverage rows require seven standardized data points to ensure consistency across plan types, which are reflected in the table below. The Entry ID is a unique alphanumeric tag assigned to each line item at the time of logging, to eliminate duplicate entries and simplify cross-referencing. The Plan Type field specifies which of the three core plan categories the entry applies to, to avoid misfiling across separate folder systems. The Incurred Date field lists the exact date the spend, transaction, or service was completed, rather than the date the charge posted to your account, to align with plan eligibility windows. The Coverage Status field is marked either Covered or Uncovered, with no intermediate statuses permitted to prevent ambiguity during reconciliation. The Eligibility Verification ID field lists the official identification number for the pre-approval, coverage confirmation, or administrator notice that verifies the status assigned to the entry. The Reason Code field is only populated for Uncovered entries, using a standardized code set defined by your plan administrator to explain why the entry does not qualify for coverage. The Supporting Document Folder Path field lists the exact location of the physical or digital supporting documents for the entry, so any reviewer can locate proof of eligibility or ineligibility in under 60 seconds.
